In the U.S., fake vaccination cards purportedly issued by the Centers for Disease Control and Prevention have appeared for sale on sites such as Amazon, eBay and Etsy. In May, officers arrested a bar owner in California for allegedly selling fake vaccination cards costing $20 each. The alleged perpetrator was charged with identity theft, forging government documents and falsifying medical records.

New York has an app now. Excelsior Pass I don't know if its being used now. Probably be harder to forge than passports. They'd probably make it like an actual passport for security purposes but don't know if that would happen anytime soon.

Yes, it is obvious that this will happen, just as driving licences, proof of insurance, passports and other similar documents are forged. The question not answered here is how serious an issue it will be - and that will depend on the design of the cards and the willingness of otherwise law-abiding people to take part in criminal activity.

My guess is it will only be a fringe problem. For instance the QR codes the UK NHS issues will take quite some expertise to forge and, given that they expire once a month and need to be renewed, I just don't believe the financial rewards for the forgers will be sufficient for the effort.
