Good point. Unless they're sterilising after each use and taking care to keep the scar clean during the healing process, then there's a good risk of infection.

The burn sterilises the burn.
I've had to burn myself many times over about fifteen years now, to kill off lesions.
Ok, I don't use heat, I use cold, and plumber's freeze spray can deliver a nice tasty burn in about 20 seconds. I prefer nitrogen, it works more quickly, but its expensive. You have to be careful that the freezing gas doesn't wrap round limbs to burn the other (perfectly healthy) side.

I don't think there's too much 'high-risk' of infection though.

Are they ever going to be in pain for three weeks +.
