They perhaps value the momentary joy (mental health) they find through smoking more than longer-term physical health.

They're bored. Least the one's I've asked. It passes time. They also assume they will win the game of Russian Roulette they play with their health. My younger sister died at 46 because of lung cancer. My mother a smoker her entire life is still going strong at 77.
P.S. Shouldn't this be under a health forum?
