I am old enough to have had a nanny who taught us such things.
Later it was down to the various house masters at boarding schools I attended. Only rarely did my mother need to step in.
The severest critics were fellow pupils.

My mother since my father spent most of the week away at work. She was fairly strict with me and my siblings but I suppose with four children she didn't really have a choice. I certainly notice the general lack of manners in society and feel that on average good manners are in decline.

People just don't seem to think that they need to show others any consideration and new technologies such as mobile phones with speaking (Really?!) make rude behavior all the easier.

I don't believe that these individuals are neccessarily bad people since it's often the case that they simply don't realise that their behavior isn't acceptable. There are however some people out there who are abundantly aware how their behavior affects others and carry out regardless because the don't care or perversely get enjoyment out of annoying others.

Manners exist on a spectrum, on one end there are those actions and reactions that lie heavily in customs and traditions, and are such quite arbitrary. Those, at least applicable to my experienced culture were taught to me by my parents, and through a process of trial and error.
On the other end of the spectrum there's manners which are more rooted in simple empathy and self awareness. These i feel i have acquired on my own or have as part of my nature by default, however my nature is in part derived from my upbringing, and so is in a certain respect attributable to my family, parents etc in a secondary degree.
