Yes blanks can kill if they are close enough. I am pretty sure that the requirment is that the debris from a blank (wad or parts of cartridge) can not penetrat a piece of
paper from 5ft and beyond.
It was a blank that fired but the modified bullet was lodged in the gun when Lee was killed.
Why after all these years later have they not completely removed the possibility for such tragedies?

We can put a man on the moon but not make a realistic movie prop gun incapable of firing a projectile.

I suspect in the near future even blank rounds won't be used and all the flashes and bangs will be computer generated.

Both things are susceptible to failure, eg, Apollo 13.
Perfection is impossible.
But movie sets can be safer if those running things
dedicate themselves to this. I've elaborated on how
they can achieve a much better record.

And for the most part, I do agree with you. If a weapon is going to be handled, the handler ought to be trained in its safety.

But since we are talking movie sets, it would seem wise to simply use firearms incapable of actual projection of material.

And the recoil will be CGI.

Or simulated.
