Life happens. Few people go through life unscathed. Skin stretches, it wrinkles, and ink fades. I'm sure this guy has interesting stories to tell, though

Agreed. But if you're worried about that, there are some "safe" areas to tat, and areas to avoid. Stomach being a bad placement. But back, hands, feet, forearms. Tend to stay similarly shaped.

I've known that some that bulk up over medical reasons(meds or other conditions).

Yes, you're absolutely right. And it should be considered if one is concerned about tattoo placement and later weight gain.

Also best to wait to get tattooed until after one has lost any significant amount of weight they may be trying to.
