It sounds bad but I think I know what he is trying to imply.
1. His sister is pregnant with her husband.
2. The man who will be a dad means that he is pregnant with his wife.

I'm of course not privy to what women tell each other about the experience but from the little I know many women seem to look forward to having a baby and tolerate all the discomfort and pain that goes along with pregnancy and delivery. But that's far from enjoying pain and discomfort.
I enjoy my children, but strongly feel their dads ought to have carried them.
Sam wanted a fourth, and I told him that's fine so long as he's the pregnant one this time.
Nice try, but you probably had to press the keys or screen really hard while walking backward on a busy street with lots of noise in heavy, torrential rain during a complete solar eclipse while answering questions and fighting off aliens. If you ask me, that is heavy labor.
