I know they aren't intersexed physically or in terms of chromosomes, but to me if you identity as male but aren't physically male, doesn't that also show we don't have 2 distinct sexes, that its more complicated than that?
There are intersexed people who sometimes identify as one or the other...
I admit I am ignorant about details of transsexualism, to me gender is a social construct, but gender identity is different and real. What transsexualism shows me is that there isn't two distinct sexes?
I believe it's when your breasts are growing you should avoid wearing an under wired bra, but if you have quite big boobs (unless you have a boob job) for most women they will sag.

Since there is no word for rape here, do you think you may be reading it too literally? It could be describing consensual sex, the woman didn't scream because she wanted to have sex, not that you literally have to scream as much as you possible if you're being raped or you'll be stoned to death...
